Unite Colorado is a coordinated care network of health and social care providers to address people’s social needs and improve health across communities
Partners in the network are connected through a shared technology platform, Unite Us, which enables them to send and receive electronic referrals. Unite Colorado is sponsored by Kaiser Permanente.

At The Center for African American Health, we see the health disparities that our community members face and we are striving to offer more and make connections to services that reflect the needs of community members of color. Professionally and personally, I am really excited to connect our most vulnerable community members with the care they need, increase accessibility and track outcomes. I believe that engaging in Unite Us is going to bring us together and streamline resources.

Food Bank of the Rockies believes all food-insecure individuals deserve access to nutritious and culturally appropriate food. In the last year, we've been able to serve over 700,000 individuals facing food insecurity with 54.7 million meals. We are excited to collaborate with Unite Colorado and other human service organizations to address root causes of food insecurity.

Having worked in healthcare and grassroots non-profit, this is the sort of solution we've been looking for collectively to support people seeking assistance in a more robust way. Having gone through the training and webinars, I'm confident that this Unite Us platform is both inclusive for the big and small players, as well as user-friendly.
