Unite Colorado is a coordinated care network of health and social service providers to address people’s social needs, and improve health across communities. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs and improve health across communities. Unite Colorado is sponsored by Kaiser Permanente, Building Hope Summit County, Tepeyac Community Health Center, the City of Northglenn, and the Family & Intercultural Resource Center. CORHIO is an implementation partner of the network. The network is supported by a Colorado-based Unite Us team focused on community engagement, network health and optimization, and customer success.
Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.
Unite Colorado serves partners in Adams, Alamosa, Arapahoe, Archuleta, Bent, Boulder, Broomfield, Chaffee, Clear Creek, Conejos, Costilla, Crowley, Custer, Denver, Dolores, Douglas, Eagle, El Paso, Elbert, Fremont, Garfield, Gilpin, Grand, Jefferson, La Plata, Lake, Larimer, Mesa, Montezuma, Montrose, Morgan, Otero, Pitkin, Pueblo, Rio Grande, Saguache, San Juan, Summit, Teller, Weld counties.
Our community champions include the United Way of Pueblo County, In The Weeds, The Hive, the Boys & Girls Clubs of the San Luis Valley, and the Health District of Northern Larimer County.
CORHIO is an implementation partner, providing local expertise and insight into the Colorado market. CORHIO introduces Unite Us to potential network partners in the community and to any of their covered entity partners who want to join the network and have patients who would benefit. CORHIO may also distribute Unite Us licenses to select covered entities.
Network partners are now able to send and receive secure, electronic referrals in Arizona, Arkansas, Colorado, Illinois, Iowa, Kansas, Louisiana, Minnesota, Mississippi, Missouri, Montana, Nebraska, Nevada, North Dakota, Oklahoma, South Dakota, Texas, Wisconsin, and Wyoming through the Unite Us Platform, which ensures communities and individuals can seamlessly access much-needed resources and services across state lines.
